 Abstract: hyperesthesia of hard tissues of the teeth (HHTT) is a frequently occurring complaint of patients, which, as a rule, leads them to an appointment with a dentist. Most often, hyperesthesia manifests itself as a sharp or moderate pain when exposed to various factors: chemical (sour foods), thermal (most often it is cold water, ice cream, a sharp change in cold drinks or food to warm) and mechanical (toothbrush, toothpick).

Pregnant women belong to a group with a high risk of dental diseases, which is caused by changes in hormonal levels, mineral metabolism, saliva pH, acid-base balance, toxicosis, vitamin D deficiency and insufficiency during pregnancy and after childbirth.

 Aннотация: гиперестезия твердых тканей зубов (ГТТЗ) – часто возникающая жалоба пациентов, которая, как правило, приводит их на приём к врачу-стоматологу. Чаще всего гиперестезия проявляется как резкая или умеренная болезненность при воздействии различных факторов: химические (кислые продукты), термические (чаще всего это – холодная вода, мороженое, резкая смена холодных напитков или пищи на теплое) и механических (зубная щетка, зубочистка).

Беременные женщины относятся к группе с высоким показателем риска возникновения стоматологических заболеваний, что обусловлено изменением гормонального фона, минерального обмена, рН слюны, кислотно-щелочного баланса, проявлением токсикоза, дефицитом и недостаточностью витамина D во время беременности и после родов.

Abstract: in the structure of morbidity of residents of economically developed countries, diabetes mellitus (DM) occupies one of the first places. The purpose of the study: To study and evaluate the immune system indicators of patients with wound infection on the background of diabetes mellitus in comparison with the data of healthy individuals. 153 patients with purulent-necrotic soft tissue lesions on the background of diabetes mellitus were involved in immunological studies, of which 97 men (63.3%) and 56 women (36.7%). At the same time, systemic inflammatory reaction syndrome was detected in all the studied patients. Analysis of the research results showed that in the examined patients the inflammatory process was strongly developed, associated with a sufficient number of pathogens in the infected focus and the development of systemic inflammatory reaction syndrome in patients, which in our opinion aggravates the course of the pathological process, and the inflammatory process in turn leads to an aggravation of the course of wound infection.

Аннотация: в структуре заболеваемости жителей экономически развитых стран сахарный диабет (СД) занимает одно из первых мест. Цель исследования: изучить и оценить показатели иммунной системы больных раневой инфекцией на фоне сахарного диабета в сравнении с данными здоровых лиц. В иммунологическом исследовании приняли участие 153 больных с гнойно-некротическими поражениями мягких тканей на фоне сахарного диабета, из них 97 мужчин (63,3%) и 56 женщин (36,7%). При этом синдром системной воспалительной реакции выявлялся у всех обследованных больных. Анализ результатов исследования показал, что у обследованных больных сильно развит воспалительный процесс, связанный с достаточным количеством возбудителей в инфицированном очаге и развитием у больных синдрома системной воспалительной реакции, что, по нашему мнению, отягощает течение патологического процесса, а воспалительный процесс, в свою очередь, приводит к отягощению течения раневой инфекции.

Abstract: in the structure of morbidity of residents of economically developed countries, diabetes mellitus (DM) occupies one of the first places. There is still a discussion in the literature about the choice of the optimal method of anesthesia for operations performed for diabetic foot syndrome. The aim of the study is a comparative study of the effect of general, epidural and conduction anesthesia on the dynamics of changes in cytokine levels in patients operated for diabetic foot syndrome. The study included 157 patients diagnosed with diabetic foot syndrome who were on inpatient treatment in the department of purulent surgery of the Bukhara Regional Multidisciplinary Medical Center. The average age of the patients was 62.5 years. The results of the study showed that out of 51 patients of the first-control group who underwent general anesthesia, 7 patients had a cardiac arrhythmia, 4 cases had uncontrolled hypotension, 11 patients had a duration of post-acute awakening, 3 patients had a hypoglycemic condition. In 5 cases, relaxant regularization was observed, and 3 patients developed congestive pneumonia in the postoperative period.

Аннотация: в структуре заболеваемости жителей экономически развитых стран сахарный диабет (СД) занимает одно из первых мест. В литературе до сих пор ведется дискуссия о выборе оптимального метода анестезии при операциях, выполняемых по поводу синдрома диабетической стопы. Цель исследования — сравнительное изучение влияния общей, эпидуральной и проводниковой анестезии на динамику изменения уровня цитокинов у больных, оперированных по поводу синдрома диабетической стопы. В исследование включены 157 больных с диагнозом синдром диабетической стопы, находившихся на стационарном лечении в отделении гнойной хирургии Бухарского областного многопрофильного медицинского центра. Средний возраст больных составил 62,5 года. Результаты исследования показали, что из 51 больного первой контрольной группы, которым была проведена общая анестезия, у 7 больных отмечалась сердечная аритмия, у 4 - неконтролируемая артериальная гипотензия, у 11 больных - длительность постистрого пробуждения, у 3 больных - гипогликемическое состояние. В 5 случаях наблюдалась регуляризация релаксации, у 3 пациентов в послеоперационном периоде развилась застойная пневмония.

Abstract: recently, with the increase in life expectancy of the population, both in our Republic and abroad, the number of patients in need of inpatient treatment with a diagnosis of acute calculous cholecystitis, including elderly and senile patients, is increasing. Analysis of literary sources indicates that up to 27.0% of destructive processes in the gallbladder proceed latently and are diagnosed with already developed complications. The purpose of the study: optimization of methods for early diagnosis of the process of destruction in the tissues of the gallbladder in acute calculous cholecystitis. To conduct this study, 91 patients with acute calculous cholecystitis were examined after diagnosis, 57 of them were women, 34 were men. The patients were divided into 2 age groups; I-group (48) of patients under the age of 65, II-group (43) of patients over the age of 65. Our morphological studies revealed a different ratio of immunocompetent cells in the gallbladder tissue in catarrhal and destructive cholecystitis: phlegmonous and gangrenous. Thus, the obtained research results indicate that there is a clear dependence of the concentration of procalcitonin and lactoferrin in the blood on the degree of development of the destruction process in the tissues of the gallbladder, which is more pronounced in patients of group 2 relative to the indicators of lactoferrin in the blood.

Аннотация: в последнее время с увеличением продолжительности жизни населения, как в нашей Республике, так и за рубежом увеличивается число больных, нуждающихся в стационарном лечении с диагнозом острый калькулезный холецистит, в том числе больных пожилого и старческого возраста. Анализ литературных источников свидетельствует о том, что до 27,0% деструктивных процессов в желчном пузыре протекают латентно и диагностируются с уже развившимися осложнениями. Цель исследования: оптимизация методов ранней диагностики процесса деструкции в тканях желчного пузыря при остром калькулезном холецистите. Для проведения исследования был обследован 91 больной острым калькулезным холециститом после установления диагноза, из них 57 женщин, 34 мужчины. Пациенты были разделены на 2 возрастные группы: I группа (48) больных в возрасте до 65 лет, II группа (43) больных старше 65 лет. Наши морфологические исследования выявили различное соотношение иммунокомпетентных клеток в ткани желчного пузыря при катаральном и деструктивный холецистит: флегмонозный и гангренозный. Таким образом, полученные результаты исследования свидетельствуют о наличии четкой зависимости концентрации прокальцитонина и лактоферрина в крови от степени развития процесса деструкции в тканях желчного пузыря, более выраженной у больных 2 группы. относительно показателей лактоферрина в крови.
